Carpal Tunnel Syndrome

Carpal tunnel syndrome is a painful condition of the wrist area caused by compression of the median nerve. It is more common in elderly and women.Patients have a sensation of “ pins and needles” at the fingertips especially in Thumb, index and middle finger. Numbness and pain affecting sleep at night is seen in severe cases.Early diagnosis and treatment can prevent damage to your muscles and nerves.

Ganglion Cyst

Ganglion cyst, a fluid filled soft swelling is commonly seen around the wrist or fingers. It is due to wear and tear changes in the joint capsule. These are generally painless but pressure can cause discomfort. Treatment includes observation, aspiration or excision. Excision or removal is recommended in cysts that are painful or increasing in size.

Trigger Finger And Trigger Thumb

Trigger finger is a common condition with symptoms of bending of fingers or locking of fingers with a clicking sound or painn when straightening the finger. This is due to inflammation of the tendon lining in the fingers. Treatment includes non-surgical options like splints or steroid injections and surgery for severe recurrent conditions.

De Quervain's Tenosynovitis

DeQuervains Tenosynovitis is a  painful condition that affects the tendons on the thumb side of the wrist .Repetitive hand or wrist movements can exacerbate the condition. It is a common source of wrist pain in women who are pregnant or just gave birth. Because it seems to affect more women particularly during the postpartum period it is also called as mommy’s thumb or mommy’s wrist.

Hand Infections

Hand infections are common and can affect all age groupsand can cause severe problems even after the infection has subsided. Early and aggressive treatment can prevent complications. Early infections can be treated with nonsurgical treatment like antibiotics and wound care.Common types of hand infectionsarebite wounds, paronychia (nail fold infection ), felons, flexor tenosynovitis, cellulitis, osteomyelitis etc.

Peripheral Nerve Tumours And Compression Neuropathies

Growths or lumps that develop in or close to nerves are called peripheral nerve tumors. The tissue strands known as neves are responsible for carrying information from the brain to the rest of the body. The muscles that allow you to move, blink, swallow, pick up things and do other tasks are controlled by peripheral nerves. Most are benign. Even non cancer nerve tumors need treatment as they can press on nerves and cause pain or nerve damage with loss of function. Specialist care and early diagnosis is important in the treatment of nerve tumors.

Claw hand

Clawhand causes the fingers to bend towards the wrist. It is also referred to as ulnar nerve palsy. It can make it hard or impossible for patients to straighten all the fingers. The stuck flexed fingers gives claw hand its name. Claw  hand may be the result of certain disease or trauma or they may be congenital defects ( defects present at birth). Treatment includes nonsurgical treatment like stretching exercises, splints,  physical therapy. Surgery includes nerve repair, tendon release, grafts or tendon transfers.

Hand swellings and tumours

Any buldge or bump in the hand irrespective of its source is a tumour. Hand tumors can develop on the skin surface like a mole or Wart, or under the skin in the soft tissue or even bone. Since the hand contains so many different types of tissue (eg skin, tendon, bone, ligaments), variety of tumours can develop like lipomas, gaint cell tumors, neuromas, fibromas, epidermal cysts etc. Treatment depends on the cause of the lump and ranges from immobilisation, aspiration, physical therapy to surgery and removal of the lump.

Hand fractures

Hand fracture happens when one or more of the bones in the hand break or shatter due to direct impact or falls. Most often nonsurgical treatment like wearing a splint, cast or buddy tapesis sufficient to repair a hand fracture. Surgery may be required to realign the bone fragments in severe fractures.

Sports injuries of hand and wrist

Hand and wrist injuries may make sports activities challenging on and off the field. Early intervention is very important. Some of the sports related injuries include Boxers fracture (knuckle bone fracture), finger Jams, Golfers elbow (inflammation of tendon), mallet finger (unable to straighten fingertips), wrist ligament tears, sprained finger etc.

Rheumatoid hand and wrist deformities

Rheumatoid arthritis (RA) is an autoimmune disease that damages joints and ligaments when the immune system attacks the body’s own tissues. The effects of the disease are often noticeable in the hands of people with RA. In addition to being painful these changes in the hand can make it difficult to perform daily tasks. Common deformity in RA includes Boutonnieres deformity, swan neck deformity, Z deformity of thumb and rheumatoid nodules. Treatment includes occupational therapy like exercises and splinting. Surgery may be required to correct severe hand deformities.

Congenital hand anomalies

Hand deformities present at birth can be in various forms from undergrowth to overgrowth or from absence of digits to extra digits. Surgery gives the best solution for congenital hand deformities to improve function and appearance.

Hand And Wrist Osteoarthritis & Contractures

Osteoarthritis of the hand is a slowly progressive joint disorder and primarily affecting load-transmitting joints. Pain, stiffness, swelling and difficulty in moving the joints are the symptoms. Severe forms may even present with deformities. Most often treatment is non-operative like non-steroid and anti-inflammatory drugs, hand therapy, splintage and activity modifications. Surgery is recommended in patients with severe deformity and persistent pain.

Contracturesare painless condition with one or more fingers bent towards the palm and cannot be straightened. This can be following burns, trauma or congenital or hereditary conditions like the Dupuytrens contracture. Surgery is the best option to correct contractures.

Cerebral palsy – spastic hand deformity

Children with spastic CP often have marked hand involvement with flexion deformity of wrist and fingers. Treatment can range from therapy, bracing to surgical soft tissue release or osteotomies depending on the severity of the deformity and difficulty in maintaining personal hygiene.
